***MANDATORY IMPACT CONCUSSION EXAM***

Prior to trying out for spring sports, ALL students must have been or need to be tested on the IMPACT system.

You will not be allowed to try-out if you do not or have not had the IMPACT test. Please call the Athletic Trainer, Steve Keefe, at 508-384-1000 ext. 307 if you have any questions or need to check if your student needs to be tested.

Physical Exams - No student may particpate in any practice, tryout or game this Fall until a physical exam has been submitted to the KP Main office. In order for the exam to be valid for the Fall season it must have a date of July 20, 2011 or more recent. This date is NON-NEGOTIABLE, the Fall season begins August 20, 2012, if you do not have an up-to-date physical in our school files by the first day of tryouts, you will be ineligible to tryout.

Tier I – Boys' and Girls' Cross-Country	$
Tier II – Boys' and Girls' Soccer, Cheerleading, Girls' Field Hockey, Football, Girls' Volleyball	$
Tier III – Golf	$

1st Student pays full price of most expensive sport

2nd Student pays 1/2 price of next most expensive sport

3rd Student pays 1/4 price of the least expensive sport
