STUDENTS WITH HEALTH ISSUES
If your child has a potentially life threatening food allergy or is allergic to bees, we will need a signed Emergency Action Plan as well as an Epi-pen and doctor’s order.
If your child has a health issue such as diabetes, seizure disorder or any other health issue that could lead to emergency treatment, we will need a signed Medical Emergency Protocol and any medications and/or supplies necessary to treat the emergency.
If your child uses an inhaler, please make sure we have an A sthma Action Plan (from your child’s doctor) a s well as an inhaler at school. If you and the doctor agree that your child can carry the inhaler or keep in his/her gym locker, we will need the Inhaler Authorization Form filled out and signed by the MD. If your child has a medical condition that may require accommodations during the school day, please fill out the four page IHCP forms so that an Individualized Health Care Plan can be developed. Any health information updates for students with health issues will also be helpful throughout the school year.

You can mail the forms to KPMS attn: School Nurse when they are completed and drop off any medication and/or supplies to the secretary over the summer or on August 31 st which is the first day of school for staff. Please check expiration dates on epi-pens as most expire after 6 months.
