The Massachusetts Department of Public Health requires the following immunizations for all students entering 7 th grade:
- 2 doses of MMR vaccine.
- 3 doses of Hepatitis B vaccine (or 2 doses if adolescent has received the Recombivax HB option for adolescents).
- 1 or 2 doses of Varicella vaccine (2 doses now recommended) or physician-certified history of chickenpox disease; and
- 1 booster dose of Td or Tdap (if it has been more than 5 years since the last dose of DTaP, DTP or DT was given).
We also require a copy of a current physical exam for all 7 th graders. You can fax this information to (508) 541-3467 or mail it to the address above. If you have a medical or religious exemption from immunizations, please send me a copy.
