Dear Parents and Guardians of King Philip, Norfolk, Plainville & Wrentham students
King Philip, Norfolk, Plainville and Wrentham Schools are pleased to be partnering with CIC Health to offer the two dose Pfizer COVID-19 vaccine to interested students 12 and older, family members and school staff. Though Pfizer is the only vaccine approved for youth under age 18, CIC will also have the one dose J&J Janssen COVID-19 vaccine available for adults 18 and up who prefer to receive it. The vaccine clinic, open to families from all four districts, will be held at King Philip High School Field House on Tuesday, June 15th from 3-7 pm for the first dose, and on Tuesday, July 6th from 3-7 pm for the second dose.
If you would like to receive the COVID-19 vaccine at our regional clinic at King Philip High School, please complete this registration form in advance of the clinic. Alternatively, the parent may accompany their child to the clinic on Tuesday and register and provide consent for them on site.
Parental consent is required for minors to receive the vaccine. The parental consent form is built into the registration form above. For high school students, please consider listing your child’s cell phone number on the form, so that they may receive text message reminders about their appointment. High School students may attend the clinic without a parent/guardian, as long as their registration form is completed in advance.
We also invite other interested family members or staff who have not yet received the vaccine to participate in this clinic. The clinic can accept walk-ins during the scheduled hours; however, advanced registration will assist CIC in preparing for adequate supply and staff, and will support an efficient clinic. CIC will also assist you in scheduling the 2nd dose appointment for the Pfizer vaccine at the first clinic.
Due to HIPAA regulations, CIC Health will not share information about who is registered for the vaccine with school administrations.
